For a professional boxer/kickboxer/mixed martial artist
who is 36 or older, or who will turn 36 during this calendar
year; and/or who has not fought a
professional fight in the last 36 months; and/or has fought over 425
professional rounds and is not under suspension in another
state.
**For the year 2012 if you were born in 1976 or earlier

Comprehensive Physical Examination Report Professional
Boxer/Unarmed Combatant
including an original laboratory report with the fighters
name and the date the HIV test and Hepatitis B Surface
Antigen and Hepatitis C Antibody tests were taken. The HIV test
must be done within 30 days of submitting all requirements to become licensed.
The Hepatitis B & C tests can be done within the calendar year.
- Dilated
ophthalmologic examination by
an OPHTHALMOLOGIST only - This must be done more than 24 hours before the
fight.

MRI of brain without contrast and MRA cerebral
circulation - If either previously
done,
forward results for review. This is a one time requirement, unless otherwise
ordered.
If any of
the testing is abnormal or questionable an appropriate
specialist will be consulted. Any and all additional
testing and referrals deemed necessary by the original
examining physician should be obtained. The additional
tests will be valid for a two-year period unless extenuating
circumstances occur. Once the commission physician has
reviewed the medical data, and your record and ability to
box has been determined, the commission will vote at a
public meeting on whether or not to issue a license.
Therefore, please submit all your required medical data to
the Commission as soon as possible, and definitely no later
than two (2) business days before the public meeting on your
application.
